Yes. You can write your own letter instead of using an official form.

Your notice must include:

  • โ€ขname, date of birth, and grade level of each child
  • โ€ขlocation of the home school
  • โ€ขcore curriculum to be offered
  • โ€ขproposed schedule of instruction
  • โ€ขqualifications of the parent-teacher
Practical tip: Many districts provide a standard form, but a simple written letter suffices. No approval is required. Filing the notice is sufficient.
